** PLEASE READ**

Contract Agreement: By purchasing, client is agreeing to access to all IN-STUDIO classes per month at Underground Barre for a minimum of 3 months at a discounted rate of approximately $5.10/class

Contract Duration and Automatic Renewal: Contract activates the day of purchase and continues for three (3) months from the contract start date. After the 3 months, a month-to-month contract will automatically renew, until canceled by the client. The membership will remain active unless canceled by the client by providing Underground Barre + Fitness with at least fourteen (14) days of written notice, emailed to the studio. If client is scheduled to be billed within 2 weeks of cancellation notice, they will be charged one additional time following the cancellation notice.

Skip a Month: I understand and agree that I may skip a month in my contract for a minimal fee of $25/month. I may skip a maximum of two (2) months, annually. I understand I must request a “skip” in writing at least fourteen (14) days before my scheduled monthly draft.

Reservations for class are first come first serve. I acknowledge that I am not guaranteed a spot in any class as a result of an unlimited class agreement. I also understand that if I am registered for a class and do not cancel my class within the sin (6) hour cancellation policy, my credit card on file will be charged a $15 late fee/no show charge.

Failed Payments: Clients will be notified by the studio if the authorized automatic credit card does not authorize. Failed payments, or declined Credit/Debit Cards will be charged a $25 late fee if not resolved within 10 calendar days, and will repeat every ten (10) days. Underground Barre reserves the right to change the price of Monthly Auto Pay packages, and will give clients 30 days notice if any changes, with an opportunity for clients to opt out of month-to-month auto charge.
