This visit is for patients who have already gone through an Initial Evaluation in the last 3 months or less. The visit must be pertaining to the injury or problem you were last Evaluated for. 

If it has been greater than 3 months since your Initial Evaluation please reach out to your provider to determine if a new Evaluation is needed.
