Client Web App: Signing a waiver

This article provides instructions for clients to use the Client Web App to sign a waiver. Clients can sign waivers for themselves or on behalf of minors whose client profiles are linked as a relationship. A minor must have their FIRST NAME, LAST NAME, EMAIL, DATE OF BIRTH, and HOME LOCATION completed on their profile before a waiver can be signed for them.

In this article

    To sign an online waiver from the Client Web App:

    1. If you are not automatically prompted to sign the online waiver:
      1. In the top menu, click My Profile.
      2. Click the Waiver tab.
    2. Use the mouse to enter your signature in the box labeled SIGNATURE OF PARTICIPANT AND/OR LEGAL GUARDIAN/PARENT OF PARTICIPANT UNDER 18.
    3. Click I Agree to Use Electronic Signatures, then in the confirmation popup, click OK. The waiver is signed.

    To sign an online waiver on behalf of a minor from the Client Web App:

    1. Click your profile icon in the top right corner of the screen.
    2. From the list, click the name of the family member whose account you want to access. You will be directed to that family member’s account.
    3. Use the mouse to enter your signature in the box labeled SIGNATURE OF PARTICIPANT AND/OR LEGAL GUARDIAN/PARENT OF PARTICIPANT UNDER 18.
    4. Click I Agree to Use Electronic Signatures. In the confirmation popup, click OK. The waiver is signed.
