PAYMENT MUST BE COMPLETED AT THE FITNESS CENTRE FRONT DESK VIA DEBIT OR CREDIT ONLY. NO CASH. USE CODE ZERO TO PROCEED TO THE FORM. This membership form is intended for Online, Part-time, Co-op, On-Break, SWAC, ACE and Apprenticeship students. (SWAC students must have paper copy signed off and returned by parent/guardian if under 18). Price is prorated from the start of the semester and covers the remaining duration of the semester (Ie. Paying $54.24 Sept 1. - Dec. 31, $47.46 for Sept. 15 - Dec. 31 etc.). Students can NOT pay one month at a time, only one payment is allowed. Apprenticeship students pay one fee for duration of their term (Ie. 8 weeks, 10 weeks, 12 weeks). Membership will expire December 31st, 2025.
