Shared Club Membership for TWO immediate members of the same family and household. Enjoy NO initiation fee when you first join! A $100 initiation fee applies only if a cancelled membership is reinstated.
Includes open gym, amenities, and unlimited classes
Applicable for individuals age 15+ including: Current Spouses/Partners, OR Siblings, OR Parent/Guardian and Son/Daughter under age 26
Club Orientation and Training Safety sessions are required for applicants under age 18 prior to full use of open gym ($150 separate additional charge); Not applicable for existing CoreForm360 clients
REGISTRATION WAIVER and MEMBER CONTRACT FOR ANY MINOR UNDER THE AGE OF 18 MUST BE SIGNED BY A PARENT/GUARDIAN
