For all in-person students / participants. By registering for this class, you agree to the terms of the Waiver of Liability, Release, and Hold Harmless Agreement found below. By registering to attend an in-person class or enter the Tai Yim Kung Fu School premises, you are confirming that following statements are true: You have NOT been diagnosed with COVID-19 within the last 15 days. You have NOT been in contact with anyone who has been diagnosed with COVID in the last 15 days. You have NOT traveled outside the continental United States within the last 14 days or been in contact with anyone that has. You have NOT been experiencing any of the following symptoms within the last 7 days: cough, shortness of breath, headache, fever, chills, sore throat, diarrhea, body aches, new loss of taste or smell, nausea or vomiting For the Security and Safety of the Tai Yim Kung Fu School Students and Staff, if any of information described above applies to you, please DO NOT come to the school. Waiver of Liability, Release, and Hold Harmless Agreement   I wish to receive instruction in the art of traditional Hung Fut Kung Fu at the facilities of the Tai Yim Kung Fu School (the “School”). I understand that since Grandmaster Tai Yim is on a temporary sabbatical and is not currently present to supervise classes, instruction will be provided through  the Masters he appointed and, under the supervision of the Masters, through other instructors who have been approved by Grandmaster Tai Yim. Due to local business requirements, the Masters have formed an entity to carry out these functions on behalf of Grandmaster Tai Yim entitled the Ninth Generation Hung Fut Masters LLC (“Ninth Generation”). During Grandmaster Tai Yim’s sabbatical I will be a student of Ninth Generation. Upon Grandmaster Tai Yim’s return, I will automatically become a student of the School. As such I certify that, as a prospective student of Ninth Generation, I am at least 18 years of age or, if the student is under 18, that I am the parent/guardian of the student and have signed this Waiver of Liability and Hold Harmless Agreement on behalf of and as parent/guardian of the student and all references herein to “I”, “me” or my” shall constitute both references to the student and to me as the parent/guardian of the student. To the best of my knowledge, I am in good physical condition and fully able to participate in the Kung Fu/ Tai Chi training for which I have enrolled, as a student of Ninth Generation, at the facilities of the School.  Notwithstanding the precautions that will be taken by the instructors and employees of Ninth Generation to ensure my safety and the safety of all other students with respect to both martial arts training, and for the time being, the threat of the spread COVID-19, I am fully aware of the risks and hazards connected with the participation in any form of martial arts training, including physical injury, and I nevertheless hereby elect to voluntarily participate in said training by the instructors and employees of Ninth Generation at the facilities of the School.  I hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE, Tai Yim Kung Fu School, Ninth Generation Hung Fut Masters LLC, and their respective instructors, officers, servants, agents, and employees (hereinafter referred to as RELEASEES) from any and all liability, claims, and causes of action whatsoever arising out of or related to any loss, damage, or injury that may be sustained by me while participating in martial arts training, or while on the premises of the School where the training is being conducted.
