Olive & Ark – Mommy & Me Class BY BOOKING THIS CLASS - I AGREE TO THE FOLLOWING:  Liability Waiver, Assumption of Risk & Media Consent Agreement I, the undersigned parent or legal guardian, understand that participation in the Mommy & Me class at Olive & Ark involves activities that may include movement, play, use of equipment, and interaction with other children and adults. I acknowledge that participation in these activities carries inherent risks, including but not limited to minor injuries, slips, falls, or accidental contact. I voluntarily assume all risks associated with participation for myself and my child. I agree to release, waive, and hold harmless Olive & Ark, its owners, employees, contractors, and representatives from any and all claims, demands, actions, or liability arising out of or related to participation in the Mommy & Me class, except in cases of gross negligence or willful misconduct. I understand that I am responsible for supervising my child at all times during the class and that Olive & Ark staff are not acting in a custodial or supervisory capacity for children during Mommy & Me sessions. I affirm that both myself and my child are physically able to participate and do not have any medical conditions that would prevent safe participation. I understand that this class does not include Olive & Ark Daily services, lounge pass or childcare with any staff members other than the direct teacher I am attending the class with my child with. I understand I am fully responsible for my child at all times while on the premise of 6005 Edmondson PIke Nashville TN 37211 Photo & Video Consent I grant permission to Olive & Ark to photograph and/or record video of myself and my child during Mommy & Me classes for use in marketing, promotional materials, social media, website content, and other business-related purposes. I understand that these images or recordings may be used without compensation and may be shared publicly. If I do not consent to photo or video use, I understand it is my responsibility to notify Olive & Ark staff in writing prior to the start of the class. By signing below, I confirm that I have read, understood, and voluntarily agree to this waiver, release of liability, and media consent.
