A re-evaluation of health and physical function and
re-assessment of initial areas of concern to determine
effectiveness of Chiropractic treatments applied. In compliance
with Standard Health Practices, this is re-eval is performed for
new injuries, annually, or after a period exceeding 90 days since
last contact to maintain current and up to date patient
